• Crew List
• Application Form COE/GOC/DCE duly filled and signed
• Certificate of Competency (COC) with the STCW 95 endorsement and GMDSS certificate for Navigational Officers of each officer applying
• Valid Medical Certificate issued not more than 6 months before the date of application by a licensed and approved physician

For issuance of Permanent COE/GOC/DCE (valid for 3 years), the following Original Documents should be Submitted before expiry of the 3 months Provisional COE:

• Application Form COE/GOC/DCE
• 3 Recent Passport Size (3.5 cm x 4.5 cm) photographs of each officer applying with his name printed on the back
